Once you have agreed upon a report writer, we will take you through the paperwork for an official agreement to work together.
The Paperwork:
- Sign and complete the Section 211 Consent for Service Form. Consent of both parties is needed unless otherwise court ordered
- Complete the Section 211 Intake Form. This provides basic information such as names of all parties, current legal orders, etc.
- Sign and Complete the Section 211 Consent to Release Information Form
- Sign and Complete the Section 211 Retainer for Services Form.
- Payment of initial retainer before report commences.
Additional Documentation:
Documentation will vary based on the type of report you require. Requirements for a Hear the Child Report or Views of the Child Report will be different from a full Section 211 Report.
