our intake process

Once you have agreed upon a report writer, we will take you through the paperwork for an official agreement to work together.

The Paperwork:

  • Sign and complete the Section 211 Consent for Service Form. Consent of both parties is needed unless otherwise court ordered
  • Complete the Section 211 Intake Form. This provides basic information such as names of all parties, current legal orders, etc.
  • Sign and Complete the Section 211 Consent to Release Information Form
  • Sign and Complete the Section 211 Retainer for Services Form.
  • Payment of initial retainer before report commences.

Additional Documentation:

Documentation will vary based on the type of report you require. Requirements for a Hear the Child Report or Views of the Child Report will be different from a full Section 211 Report.

  • Login to our secure document portal (we will provide you access)
  • Email or Upload all legal documentation and court orders to portal
  • Complete the 33 page Section 211 Questionnaire
  • Upload any documentation that is relevant to the report including:
    communication between parents
    – assessments
    – doctor’s reports
    – report cards
    – certificates of achievement, etc.
